The pieces are slowly coming together as we begin to wind down this year’s session. Talks continue on the state budget and the number of bills on the debate calendar continue to drop off as we complete our work.

Today, the state’s new Medicaid Modernization plan starts and managed care organizations (MCOs) will begin managing the care of Iowa’s 560,000 Medicaid recipients. This is a big change for the state as we move from a fee-for-service model to a coordinated one, but I’m optimistic that this change will be a benefit to the patients that rely on high quality healthcare from Medicaid.

So why is the state switching to a modernized Medicaid system? The reason is twofold.

First, the current Medicaid system is bureaucratic, inflexible, and unpredictable in cost. As the second biggest program in state government, behind K-12 education, the cost of Medicaid has skyrocketed over the last decade. According to the nonpartisan Legislative Services Agency (LSA), yearly state revenue grew by 2.6% between 2006 and 2015. During that same time, funding for schools grew yearly at 4.3% while Medicaid grew by a staggering 9.1%. This unsustainable growth threatens other functions of government– like education, public safety, and courts – at risk of being crowded out. The old Medicaid system simply does not operate like an efficient system should.

Second, the state’s antiquated Medicaid system doesn’t focus on making patients healthy, and simply works to treat diseases and illnesses as they occur. Under managed care, the new system will focus on preventative care to help patients lead healthier lives. This is done through coordinated care by promoting wellness measures and tackling chronic disease. These steps will help patients enjoy a higher quality of life.

Medicaid Modernization provides the state with an opportunity to control these ever-growing costs while also improving patient health.

Under the new system, benefits of Medicaid will continue to be offered. All current services available to Medicaid patients should not change and patient identification numbers should also remain the same. Patients can switch to a different managed care organization at any time for any reason between now and May 17. Following that date, patients are still able to switch for good cause, such as a provider not being signed up with an MCO. There will also be an annual open enrollment period in late fall to switch to a different MCO or remain with the current one.

As Medicaid Modernization rolls out over the next few days and weeks, please let me know if you run into any problems. I’m sure that there will be minor snags and hiccups during this transition and I’m ready, willing, and able to assist all of those Iowans who may be affected during this change.

Additionally, House Republicans have been working to craft legislation that provides a degree of legislative oversight to the Medicaid Modernization plan. This will likely be included within the Health and Human Services budget bill. House Republicans’ plan will focus on access, quality of care, and quality of outcomes to ensure that patients continue to receive the high quality healthcare that they expect.

Any change, especially one of this magnitude, is always difficult in the beginning. However, I’m optimistic that this is the right approach to make Iowans healthier and provide predictability to the Medicaid budget. I’m aware that this transition has been a point of controversy, but now is the time to come together and work to make this as successful as possible for those that count on Medicaid services.

No, the statement stands as I posted it. Check around yourself and you will see the Democrats are behind this just like I said. But, if you are a Democrat, you won’t be able to believe the truth anyway. I should know as this very thing happened to me.

Privatization of Medicaid in Iowa is Terry Branstad’s deal. You are entitled to your opinion but, in this case, you are dead wrong.

I’m severely disabled and have been since I was two years old I have had multiple operations three of them and then bring operations there is no way that I am able to work. My left leg drags my left arm is completely in mobile and cannot hold anything I’m lucky to be able to turn it the brain operations that I had were thanks to Dr. Frederick Mayer and Dr. Gregory casino, I only would’ve been able to have these if it would’ve been for the Mayo Clinic in Rochester I was told that I would no longer be able to go there. I am now scrambling to find A neurologist that is able to treat me and the difficult and severe case that I am if I had a choice to work I would! My life has not been easy ! To have something taken away from you such as the doctor that you’ve seen for almost 35 years on a special case is in my opinion asinine The government should not be involved in this and they certainly should not have taken the disabled and difficult cases away from the Mayo clinic and the doctors that treat the hard cases ! For money and greed and that’s what it boils down to

Do you whiney people get it ? Do you know who the majority of the people who receive Medicaid are? The disabled: Mentally, Physically and chronic and persistent mentally ill, elderly and frail seniors on the minimum of social security– some as low as $ 430 per month. Some are of the working poor class, working at companies that call 31 hours a week part time so they do not get any healthcare or other benefits. Private corporations drive this, so they can turn a better profit. That is what America is .. we are a capitalistic society, and it is what it is. People who have medicaid get just as good, if not better health care. They pay nothing out of pocket. I have insurance through my employer, but I still pay a portion for the insurance. I have copays at the doctors office, and at the pharmacy. Patients with medicaid pay nothing. Children and elderly are the people who need medicaid. What we have is a problem with some people not wanting to work, and would rather collect any government assistance they can get. Our system is broken.

Also, Mayo Clinic has not reached an agreement with any of the 3 providers so those who depend on care from Mayo Clinic will have services cut.

Not true. Several of the Mayo Clinics have reached agreements. The Main May Facility in Rochester has not for “routine” exams but will make arrangements for specialty services.

Just to improve on your stupidity, here is some more info for you. Mason City Clinic along with Mercy Hospital has agreements with Medicaid as well as the Mayo Clinic in Lake Mills. Ia. Wake up and get educated before you look any dumber than you already do.

Iowa is not signing a contract with Mayo. Minnesota and other states have or will be. To get accurate information just call your representatives. Also, yesterday was a disaster. Mercy finally signed 2 contracts, Amerigroup and Amerihealth. But not, the third one thru United Healthcare. So there were people yesterday who could not get services, including Meals on Wheels. This is a major disaster. I also have an aquaintance who needs a shot each month. She could not get shot this month, due to Mercy scheduling multiple surgeries for Senior Citizens on Medicaid, so they can get paid the current amount. The only ones getting anything out of this are the 3 Managed Care companies, with CEO’s raking in salaries over
$ 500,000 to $ 1,000,000 per year. That is what Privatization means. The rich get richer and the poor get poorer.

This is pretty laughable. Upmeyer says House Republicans “..working to craft legislation that provides a degree of legislative oversight to the Medicaid Modernization plan. ” That is true now but when Democrats first proposed it, 90% of Republicans opposed it because Terry Branstad opposed it. Branstad said adding more oversight was not cost effective and what they originally had was adequate. But, with much public outcry, his fellow Republicans realized their future as legislators was at risk so they decided to give in to public opinion and common sense, and agreed to come up with some sort of oversight.
She also says “All current services available to Medicaid patients should not change”. Notice she says SHOULD NOT, not will not. That gives her an out once the managed care starts and services are cut or reduced. Republicans jumped all over our President when he said similar things about the Affordable Care Act which some call Obamacare because he said “will not” when he should have said “should not”. We will see how Linda backtracks once the problems start.
