Tax booklets no longer available at library

The Iowa Department of Revenue announced this fall that they will not be sending out individual tax booklets for the year 2011. Consequently, the Mason City Public Library is unable to provide paper tax booklets for state taxes to our patrons.

If you still want to file a paper form, your options include:

• Request a paper form by email at IowaTaxForms@ Iowa.gov
• Request an online fillable form at www.iowa.gov/tax
• Request a paper form by phone at 1-800-532-1531
• Request a paper form by mail:

Iowa Department of Revenue
PO BOX 10460
Des Moines, IA 50306-0460

IDR states that due to limited availability, only one booklet will be provided to each customer.

You may choose to file electronically. Please contact the Iowa Department of Revenue in early January for more information at: http://www.iowa.gov/tax

MCPL will gladly print off forms for you for $.20 per page.
