Numerous health violations found at Mason City OP after eight customers claim to fall ill

A complaint was called into the Cerro Gordo County Public Health Department concerning eight people that became sick after they had eaten at the Other Place in Mason City on December 10, 2010 around 8:00 pm. The ensuing inspection that was done on December 13th found that “the establishment has numerous violations and is excessively dirty.”|The following are complaints the Cerro Gordo County Department of Public Health investigated at restaurants in Mason City, Iowa, sometime in 2010. Some are several months old but the information is still relevant and available to the public. We may have missed some complaints as the list of restaurants is very long. The Other Place (Mason City)
A complaint was called into our office concerning 8 out of 16 people became sick after they had eaten on December 10, 2010 around 8:00 pm. at the OP in Mason City. Foods eaten, but not limited to pizza, chicken strips, nacho’s etc. A list of employee’s working was received during this inspection. No employee’s had been sick that the manager was aware of. The establishment has numerous violations and is excessively dirty. This department was told that this facility would be remodeled for the last several years and to date nothing has been done to the prep area. Remodeling and correcting structual problems must be scheduled within 30 days and written corrections must be mailed or faxed to our office. The facility must have an extensive cleaning done by 9:00 am on Wednesday, December 15, 2010 or the facility will be closed until cleaning is complete. Ryan Fiser was present during the inspection on Monday, December 13, 2010 and was told he must have the cleaning done and violations corrected by 9:00 am on December 15, 2010.

Re-check Inspection on 12/15/2010, from 9:45 AM to 11:30 AM by Fullerton, Dixie: Remodeling and correcting structual problems must be scheduled within 30 days and written corrections must be mailed or faxed to our office. Nice job of cleaning and correcting violations, the facility must remain clean at all times and the construction issues must be fixed.

BUFFALO WILD WINGS GRILL & BAR
A complaint was called into our office on 8-27-10 concerning the re-serving of chips and salsa and that the mens room was dirty. Manager got him new chips and salsa. I spoke to the GM today and no chips and salsa are re-served and the mens room is cleaned every 20-60 minutes.

KENTUCKY FRIED CHICKEN (Mason City)
A complaint was reported to this office regarding company sick policy. The complaintant stated management required them to attend work when ill, if they could not find a replacement for their shift. Current company policy requires 2 hours notice prior to shift if an employee will be calling sick. A replacement must be arranged by employee. The current employee handbook contains all Food Code required documentation of reporting illness to the person in charge. Management stated they would not allow an ill employee to work and does not require ill employees to report to work. This was emphasised in conversation with management: ILL EMPLOYEES ARE NOT TO BE ALLOWED TO WORK.

PLAZA MEXICO MEXICAN RESTAURANT (1809 S. Federal)
A complaint came to the health department regarding re-servicing of food. The complaintant stated food was served to their table. Members of the party ate from plate. Wait staff had mistakenly brought out the wrong food. Wait staff returned to the table, remove the wrong plate of food, then served food to another table. During this inspection the management was made aware of the possible health issues this could create. Management will revisit training with staff regarding service of food.

PRETZEL MAKER (100 S. Federal)
Two complaints came to this office concerning glove use and handwashing at this facility. In both complaints glove use was a concern when rolling out dough. Facility must use gloves if rings/nail polish/fake nails are worn while rolling out dough. One complaint also stated that improper hand washing was being done in the evening shift. Proper handwashing guildelines and procedures need to be reviewed with all staff. Staff during inspection was properly washing hands.

THE PITA PIT
This office recieved a complaint from a patron that allegedly became sick a short time after consuming food from the facility. In speaking with staff all procedures are being followed to protect and maintain food safety. Proper temperatures, proper handwashing, proper dish sanitiation was all being followed at the time of the inspection.

COMMUNITY KITCHEN OF NORTH IOWA
A complaint was filed with this department regarding the following issues: kitchen workers hair not being restrained, pork product served undercooked, tables being cleaned in closed proximity to people eating. Kitchen workers hair does need to be restrained as outlined in the 2005 food code chapter 2 section 2-402.11(A). Please inform staff and volunteers when and how to restrain hair while working around food. The pork product in question is a fully cooked canned product. Talking with staff this product is brought up to the appropriate temperature of 165F prior to serving. Be sure products are being temped to meet temperature standards outline in the 2005 food code chapter 3 subpart 3-401, 3-403. Speaking with staff the tables are cleaned in a manner that should not cause any aerosolized contamination. Be make staff aware not to clean close to open or uncovered food. Use of wet floor signs was also discussed. Use wet floor signs when appropriate.

HY-VEE (2400 4th St. SW)
A complaint came to our office regarding a employee entering the restroom without removing their apron. Management has resolved the issue with the staff memeber as well as hanging signage in staff areas.
