OSAGE — Two drivers were killed and a passenger was airlifted to a hospital after two vehicles collided head-on Friday afternoon in Mitchell County.
The Iowa State Patrol said the crash happened at approximately 4:05 p.m. September 4 at Kirkwood Avenue and 440th Street near Osage.
A 2018 Chevrolet Equinox driven by 56-year-old Stephnie Lynn Lyons of Rochester, Minnesota, was traveling south on Kirkwood Avenue as a 2008 Dodge Dakota driven by 71-year-old Charles Edward Yoders Jr. of Stacyville approached from the opposite direction.
The two vehicles collided head-on for reasons that remained unknown.
Lyons and Charles Yoders were both killed. Lyons was wearing a seat belt, while Charles Yoders was not restrained, according to the State Patrol.
A passenger in the Dodge, 71-year-old Yvonne Marie Yoders of Stacyville, was injured and flown by Mayo Air Med to Mayo Clinic. She was wearing a seat belt, which investigators credited with saving her life.
The bodies of the two drivers were taken to the Iowa State Medical Examiner’s Office.
The Iowa State Patrol continues to investigate the collision. Mitchell County EMS, the Mitchell County Sheriff’s Office, St. Ansgar Fire Department, Mayo Air Med, Hoff Celebrations and Osage Towing assisted at the scene.
As of September 4, preliminary state figures showed 196 people had died in traffic crashes on Iowa roads during 2026.
