RANDALIA — Multiple people were injured and two vehicles were destroyed in a crash west of Randalia on May 14, 2026.
According to the Fayette County Sheriff’s Office, deputies received multiple 911 calls at approximately 12:50 p.m. reporting a two-vehicle crash at the intersection of 150th Street and R Avenue, about two miles west of Randalia.
Investigators determined that Kaitlyn Marie Hackley, 23, of Fayette, was traveling westbound in a beige 2005 Ford Taurus SEL when her vehicle was struck by a gray 2007 Honda Pilot LX driven by Joshua James Weltha, 42, of Hawkeye.
Authorities said Weltha was traveling southbound and failed to obey a stop sign and yield the right of way.
Hackley was trapped inside the Ford and had to be mechanically extricated. She suffered serious injuries and was transported to Gundersen Hospital in West Union. The Ford was considered a total loss.
Weltha suffered minor injuries and was transported to Sumner Hospital. A male juvenile passenger in the Honda also suffered minor injuries and was taken to Sumner Hospital.
A female passenger in the Honda suffered life-threatening injuries and was airlifted to Mayo Clinic in Rochester, Minnesota.
The Honda was also considered a total loss.
The Fayette County Sheriff’s Office was assisted by the Hawkeye Fire Department, Hawkeye First Responders, Sumner Ambulance, Fayette Ambulance, Iowa Air Care and the Iowa State Patrol.
The crash remains under investigation.
