Iowa worker killed by falling object while operating forklift; OSHA investigating

BURLINGTON, IOWA - An Iowa worker was killed as he operated a forklift and an object fell and struck him in the head, and now OSHA is investigating the incident.

BURLINGTON, IOWA – An Iowa worker was killed as he operated a forklift and an object fell and struck him in the head, and now OSHA is investigating the incident.

At 2:16 PM on September 15, 2023, an ambulance was requested to respond to 724 Jefferson Street for an injured male who was working in the building. The ambulance personnel and Burlington Police Officers discovered the victim was unconscious and bleeding from the head. Officers were told the victim was moving items with a forklift and pallet jack and was struck in the head by a falling object. The victim was pronounced dead at the scene. It is believed this incident was accidental but remains under investigation.

The decedent in the accident has been identified as 42-year-old Travis Brown from Ft. Madison, Iowa. Mr. Brown was working for a company contracted to perform work at the Busted Cup Brewery. At the time of the accident, Mr. Brown and another man were attempting to load a forklift onto an elevator when the forklift fell over and struck Mr. Brown in the head.

The events leading up to Mr. Brown’s death has been determined to be accidental and the Occupational Safety and Health Administration (OSHA) has opened an investigation into Mr. Brown’s accidental death.
