DATE OF ESCAPE: October 22, 2012
COMMENTS: Avery was placed on escape status at 11:00 pm after failing to return to the facility as scheduled. Avery was admitted to work release on September 4, 2012. This sentence began on October 30, 2002. Information as to Avery’s whereabouts should be reported to the Des Moines Police Department or Polk County Sheriff’s Department.
